Requirements

Certificate of Insurance - with additional insurance coverage under section A, the City of Chattanooga listed as the additional insured, and the City of Chattanooga as the certificate holder.
A City of Chattanooga business license.
A City of Chattanooga Master Plumbing license or a State of TN Contractor's license.
A street obstruction and excavation bond bound to the City of Chattanooga in the penal sum of $25,000.00.
License holder (QA) must sign permit
